Account book maintained by an unidentified physician in Washington, Indiana from January 1890 to March 1893. There are 264 pages of handwritten entries of services provided to patients, the date of service, the fee charged, the amount paid by the patient and the balance owed. In the front there is an alphabetized list of the patients with page numbers where each patient's account is entered. The standard charge for a home visit was $1.00. If the home visit required an "operation," the doctor's fee ranged from $1.00 to $3.00. On the inside cover is written "#1240" which is a reference to the Thelma Bingham books at the Daviess County Museum. She wrote this item was part of a collection donated to the Museum on September 27, 1986 by Martha (Lett) Ward. Item 6 in Ms. Ward's donation was described as a "Doctor's Record Book." The names in the People Field are just a portion of the individuals referenced in this book. |
