June 2, 1938 handwritten letter from Dr. Mac G. Porter (1866-1952) in Elnora, Indiana submitting a claim to Elmer R. Chestnut (1891-1955), Trustee of Bogard Township, Daviess County, for payment of the medical bills of Henry P. Thomas of Epsom, Indiana. Included were Dr. Porter's February 24 and June 1, 1938 itemized statements of medical services provided to Mr. Thomas. Dr. Porter charged $1.50 for an office call and $5.00 for a home visit. |
